Chlorpromazine-tadalafil interaction leading to refractory ischemic priapism and penile prosthesis implantation: a case report

ARİCİ,CAN | BAŞARANOĞLU,MERT | ÇAYAN,SELAHİTTİN | BOZLU,MURAT | AKBAY,ERDEM

Abstract , Murat Bozlu1 and Erdem Akbay1 Background Priapism represents a rare urological emergency characterized by persistent penile erection unrelated to sexual stimulation. Although chlorpromazine-induced priapism has been documented, the synergistic interaction with phosphodiesterase type 5 inhibitors resulting in refractory cases necessitating penile prosthesis implantation constitutes a novel clinical scenario requiring comprehensive documentation. Case presentation We report a 56-year-old male who developed refractory ischemic priapism following self-administration of oral chlorpromazine for hiccups, taken 24 h after tadalafil 20 mg for erectile dysfunction. The patient had previously experienced a transient priapism episode 10 years earlier following isolated chlorpromazine use, e...

Lipoma in the Bladder Mucosa with MRI Supported: Case Report

TURHAN,AHMET | AÇIKAL,TOLGA | AKBAY,ERDEM | YUYUCU KARABULUT,YASEMİN | MELEMEZOĞLU,DAMLA | APAYDIN,F. DEMİR

Abstract Lipomas are typically encapsulated benign tumors found in the skin, central nervous system, or gastrointestinal system. Lipomas of the bladder wall are rare tumors with limited reported cases. We present a case of a 62-year-old male patient who presented to our outpatient clinic with complaints of hematospermia. During two pelvic magnetic resonance imaging (MRI) scans conducted ten months apart and a diagnostic cystourethroscopy, a 7 mm lesion was incidentally found on the dome of the bladder and was resected for further examination. Follow-up MRI revealed that the size of the lesion remained unchanged. In spite of their rarity, bladder wall lipomas should be considered in the differential diagnosis of bladder tumors.  Keywords: bladder lipoma, hematospermia, tumor

COMPARISON OF SEXUAL FUNCTION AFTER RETROGRADE INTRARENAL SURGERY AND URETEROSCOPY: A RETROSPECTIVE COHORT STUDY

TUNÇ,VEYSİ | BAŞARANOĞLU,MERT | TEK,MESUT | AKBAY,ERDEM

Abstract Objective: Retrograde intrarenal surgery (RIRS) and ureteroscopy (URS) are widely adopted minimally invasive techniques for treat ing upper urinary tract stones. However, their comparative effects on sexual function remain poorly understood. This study aimed to compare the impact of RIRS versus URS on sexual function and identify risk factors for prolonged dysfunction. Material and Methods: In this retrospective cohort study, we analysed 360 propensity scorematched patients (180 pairs) who underwent RIRS or URS between January and December 2022. Sex ual function was assessed using validated questionnaires (IIEF5, PEDT, MSHQ for males; FSFI, OABq SF for females) at baseline and 3, 6, and 12 months postoperatively. The primary outcome was the change in sexual function scores; the se...

Evidence‐Based Treatment Ladder for Chronic Prostatitis/Chronic Pelvic Pain

BAŞARANOĞLU,MERT | AKBAY,ERDEM

ABSTRACT Objective: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) affects 2%–10% of men worldwide, yet lacks stan dardized treatment algorithms. This study aimed to establish a definitive treatment hierarchy by comparing four therapeutic modalities: alpha‐blocker monotherapy, alpha‐blocker plus antibiotic combination, alpha‐blocker plus saw palmetto combi nation, and transurethral resection of prostate (TUR) surgery. Materials and Methods: This retrospective comparative cohort study analyzed 200 adult males with CP/CPPS (50 patients per group) treated at a single academic center between October 2023 and August 2025. Inclusion criteria comprised NIH Chronic Prostatitis Symptom Index (NIH‐CPSI) total score ≥ 10 and pain score ≥ 4, with excluded bacterial infection. Pri...

Hundreds of Ileal Condylitis Stones

TURHAN,AHMET | BAŞARANOĞLU,MERT | AKBAY,ERDEM

Abstract CASE REPORT One of the long-term complications of urinary diversion, urolithiasis, is influenced by factors such as bacterial colonization, urinary stasis, mucus, and anatomical abnormalities. Stones are typically observed in the upper urinary tract, and they are rarely seen in the ileal conduit. Although endoscopic surgery can be used for the treatment of stones in the ileal conduit, it is predominantly treated with open surgery. We report a 28-year-old male patient with 282 urinary stones, the largest of which measures 8 cm, found in the ileal conduit. The case is evaluated in terms of treatment and surgical approaches.

From digital assistants to clinical partners: revolutionizing pediatric urology through large language model-powered decision support and patient education

BAŞARANOĞLU,MERT | AKBAY,ERDEM | ERDEM,ERİM

Abstract Background Large language models (LLMs) demonstrate increasing potential in healthcare applications, yet their clinical utility in specialized pediatric medicine remains inadequately characterized. This study evaluated LLM performance in pediatric urology to establish evidence-based implementation frameworks. Methods We conducted a two-phase evaluation of thirteen current-generation LLMs between January and April 2025. Phase 1 assessed clinical decision support using 180 standardized cases stratified by complexity (126 routine, 54 complex) based on European Association of Urology pediatric guidelines. Phase 2 evaluated patient education effectiveness across four common pediatric urological conditions. Performance was measured using mathematical similarity metrics and standardized ...

Prevention of infectious complications after transrectal ultrasoundguided prostate biopsy: comparison of povidone-iodine, chlorhexidine, and formalin disinfection

BAŞARANOĞLU,MERT | NEBİOĞLU,ALİ | BOZLU,MURAT | AKBAY,ERDEM

Abstract Purpose We aimed to compare the efficacy of three different antiseptic methods to determine the most effective prophylactic approach to prevent infectious complications after transrectal ultrasound-guided prostate biopsy (TRUS-PB). The methods evaluated were transrectal povidone-iodine injection (TRPI), biopsy needle disinfection with chlorhexidine, and biopsy needle disinfection with formalin. Methods Between January 2018 and January 2023, 904 patients who underwent TRUS-PB were retrospectively analyzed. All patients had prophylactic antibiotic use and negative urine/rectal culture results. Patients were divided into three groups according to the antiseptic protocol: Group 1 (n = 245) received only TRPI injection into the rectum before biopsy, Group 2 (n = 295) received only chlo...

The impact of normalized energy utilization on clinical outcomes and complications in endourological stone surgery: a prospective study

BAŞARANOĞLU,MERT | KUŞ,OKTAY | TEK,MESUT | AKBAY,ERDEM

Abstract · This prospective study evaluated the impact of normalized energy utilization on clinical outcomes and complications in endourological stone surgery. Between January 2024 and April 2025, 478 consecutive patients undergoing holmium: YAG laser lithotripsy were enrolled. Stone characteristics, energy parameters, operative outcomes, and complications were prospectively recorded. Normalized energy values (J/mm³) were calculated as total energy divided by stone volume. The primary endpoint was complication development. Mean normalized energy was 22.5 ± 13.1 J/mm³ (range: 5.8–72.4). High-density stones (> 1000 HU) and lower calyceal locations required significantly higher energy (p < 0.001 and p = 0.012, respectively). ROC analysis identified 25 J/mm³ as the optimal threshold for predic...
